How to Get Enrolled

• Click on "Enroll in OPIS Survey" , then click on "Verify" to check if the employee number is valid.
• Chose the password retrieval method (Either Email if you want to have the password back using the system email address or via Security Question if you want the system ask you a personal question in an event you forget or loose your password).
• Type your last four digits of SSN.
• Chose the Password and Retype it for checking purpose.
• Click on "Register", the system will redirect you into the system.

What to do in an event you loose your password?

• Chose "Forgot Password" in the main page.
• Type your employee number and click on "Search" bottom to validate the employee number. If you chose the retrieal method by security question the system will show you the secure question you chose and prompt you to type the rigth answer.
• Type your four last digits Social Security Number and click on "Submit" for the system
• Type your last four digits of SSN and click on "Submit"
